Participation in this survey is voluntary. Completing and submitting this survey will serve as "implied consent" that the information provided can be used by the researchers. In addition, your participation will be anonymous, unless you provide your e-mail address and the name of your institution indicating you would like to receive a report from the survey. If you do provide information about your identity and your institution, neither will be directly identified in any report from this study. Only aggregate information from the survey items will be reported. Questions about your rights as a participant in this study may be directed to Ms. Ruth Smith, Office of the Vice Provost for Research, Institutional Review Board, Temple University, N. Please complete this survey to the best of your knowledge. We recognize that in some instances there may be multiple agents within a University or College who are responsible for distance learning activities. If there are other people in your institution who deal with distance learning policies, please forward this URL (http://isc.temple.edu/schifter/survey/distance_survey_r2.htm) to their attention so their information can be included in this survey.
For the purposes of this study, distance education is considered to be any form of teaching and learning that occurs totally through technology mediated communication. What is NOT included are correspondence courses, courses where faculty drive to a remote site, and courses where faculty meet students face-to-face at any time during the course. Please keep this definition in mind as you complete this survey.
